Available Services

Ryan White Case Management

Ryan White case management services are available to Persons Living with HIV/AIDS (PLWHA) who do not have Medicaid. This program provides comprehensive care coordination and navigation/referral services. Staff members will work with you in order to help ensure access to primary and HIV medical care, support services in the community, and benefits/entitlements for financial security.

Health Education Services

Health Education services are available to all Persons Living with HIV/AIDS (PLWHA) who are interested in additional education about HIV, concurrent medical conditions, viral suppression and medication management. The staff members in this initiative provide individualized education about a variety of HIV and related topics. By learning about medication adherence, resistance, or concurrent medical conditions, you will be better able to understand ways to improve your health and manage your viral load.

Behavioral Health Education Services

The Behavioral Health Education Program, Bridges to Wellness, provides educational and referral services for Persons Living with HIV/AIDS (PLWHA) who are in need of mental health or substance use services. This program prioritizes services for individuals who not currently in care. Staff members will help you identify what type of care you feel would be of the greatest benefit. They will then provide you with personalized education and support, as well as assist you with getting to your first three appointments.

Nutrition Services

The HIV Nutrition Educators work directly with PLWHA to improve their understanding of diet and nutrition. The also teach individuals simple, everyday strategies to improve their diets and health outcomes. This program prioritizes services to individuals who are currently experiencing increased medical concerns, and clients are enrolled for up to twelve months. If you are interested in getting information about how to better manage your health through dietary improvements, please schedule a time to meet with one of our nutrition educators.

Housing Opportunities

STAP operates two housing programs for PLWHA in the Southern Tier. Both programs offer a rental subsidy, as well as single-event financial assistance, to qualifying clients with limited incomes. The short-term subsidy program also provides housing case management services, to help ensure household financial stability, access to HIV and primary medical care, and referrals to community support services.
